Healthy Weight Scheme

The Healthy Weight scheme acts as a ‘one-stop shop’ for community weight management services. The scheme is designed to offer integrated weight management services across the city, providing a seamless pathway of care, linking primary and secondary care.

The scheme links to the weight management services in clinical and community settings to offer a seamless pathway of care for the community of Brighton. It provides GP’s and other health professionals easy access to the full range of community weight management services across the city.

This scheme was developed after consultation with GP’s and other health care providers to simplify and coordinate referrals to weight management services. The PCT commissioned the Food Partnership to deliver this scheme alongside the existing clinical and community weight management services and the other referral schemes in the city.

A primary objective of the scheme is to raise awareness of weight management services available across Brighton and Hove for adults and children, making these programmes available and accessible to all.
